Welcome to the North Campus Survey Platform:

This platform is dedicated to providing a safe and anonymous space for all students to share their concerns, opinions, and suggestions for me to raise to the Board. Your voice is important, and this platform ensures that every student’s perspective is heard and valued. Additionally, this is also a safe space to share any thoughts or opinions you have on the agenda for Student’s Council and/or provide me with any feedback you have! 

Guidelines for Filling Out the Form:

1. Be Honest/Constructive: Constructive feedback helps us understand and address the issues effectively.
2. Specificity Helps: Provide as much detail as possible about your concern or opinion. The more specific you are, the better I can understand and address the issue.
3. Confidentiality: Your responses are completely anonymous to me unless you choose to leave your contact information or book a follow-up meeting with me. In both cases, concerns will still be shared anonymously during Board meetings,

What Will Not Be Tolerated:

• Hate Speech: Any form of hate speech or harassment will still be read but not addressed.
• Personal Attacks: Personal attacks or defamatory comments against individuals will not be accepted.
• Spam: Repetitive submission is allowed but keep responses to 1-3 a day. Any irrelevant submissions that do not contribute to the discussion will not be addressed.

How to Submit a Form:

1. To access the form, click on the link provided on this page to access the survey form.
2. Not all fields are required, but feel free to tailor the survey response to best reflect your concern!
3. Once you have completed the form, click the “Submit” button. Your response will be recorded anonymously.

Need Additional Support?
If you require additional support or prefer a non-anonymous meeting, you can:

• Book a Meeting: Visit the “Contact” tab to schedule a virtual or in-person one-on-one meeting with me. This is an opportunity to discuss your concerns in more detail. Your meeting with me and anything discussed will still stay anonymous but your concern will be written down with permission.
• Leave Contact Information: If you wish to be contacted for a follow-up, head over to the contacts section and leave your email address in the contact information.
